Meet one-on-one with an advocate who will request your ID, Social Security card, or Pennsylvania birth certificate for you.

For the appointment, please bring any of the following:

To show proof of identity: Birth Certificate, Social Security card, Military ID, marriage or divorce certificate, citizenship or naturalization certificate, old photo ID such as driver’s license or school ID, passport.

To show proof of residence: Utility bill, rental agreement, mortgage statement, bank statement, medical bill, W-2 form, word ID, tax return.
